Police Background Checks **UPDATED - See Below**
As a reminder, OMHA policy requires all Team Officials, including Coaches, Assistant Coaches, Trainers and Managers, to provide a Criminal Background Check as a condition of volunteering to work with children.
While some consider this to be a technicality, SMHA fully supports the OMHA’s Policy on Criminal Record Checks, as this important requirement helps to provide a safe and secure environment for SMHA families and players. A copy of the applicable OMHA Policy is available in the Risk Management Section of our website.
Also included in the Risk Management Section of our website, you will find the necessary Application Form, used by York Regional Police, required for the purposes of obtaining a Police Background Check. You can print out the form and complete it prior to attending in order to expedite your request.
Two pieces of identification will be required as part of the screening process, and the cost is $20.00 for volunteers. There are currently two locations where the Police Background Check can be obtained. The second location is relatively new (South Customer Service Unit in Aurora) – and speaking from recent experience, the staff at that location were quick and efficient, with the entire process taking less than 10 minutes to complete.
Individuals intending to volunteer for SMHA positions during the coming year are encouraged to obtain their Police Background Checks as soon as possible, to avoid any potential delays.
Once obtained, please provide a copy to SMHA Director of Risk Management, Kelly Harbridge

Background Checks for Team Officials Living Outside York Region
If you are a Team Official living outside of York Region, you will not be able to obtain a Police Background Check through York Regional Police. Instead, you will need to apply for the required Police Background Check through the Ontario Provincial Police (OPP), and the nearest detachment. Unfortunately, the OPP process is somewhat more complicated than the process used by York Region. There is a separate OPP Application Form, which is available on this site for download at your convenience (see OPP Request for Vulnerable Sector Screening Check). Before you apply in person using the OPP Form, you will see that the first page needs to be completed by someone acting on behalf of the Schomberg Minor Hockey Association (SMHA). Any of the SMHA Executive Members would be happy to complete this form on your behalf, which will enable you to apply to the OPP for the required Background Check. Should you have any questions about the OPP application process, please contact Risk Management Director, Kelly Harbridge.
